Nondisclosure Agreement (NDA)

This nondisclosure agreement is entered between the owner (Amber Buti of Butify) and you (The Client) below.

The Owner acknowledges the Client may receive information regarding the owners business practices or other properties that may be considered confidential. Therefore, the parties are in agreement to the following terms and conditions of this agreement:

Confidentiality

For the duration of this NDA, any information the client may come into contact with that is not public knowledge and is made only available through contact with the owner shall be deemed confidential information.

For the duration of this NDA, any information the owner may come into contact with that is not public knowledge and is made only available through contact with the client shall be deemed confidential information.
Example but not limited to: personal affairs, feelings, or events unless showing signs of harm to yourself or others
The client, should use common judgment when deciding if information is confidential and above all shall request approval from the owner prior to the release of any questionable information

Term of Agreement

The obligations of this nondisclosure agreement shall survive indefinitely including through termination or conclusion of this relationship.

Additional Terms *

I agree the terms of the NDA which includes sharing the owners address with anyone. If referring a friend only the owners phone number or social media may be shared.

Installments & after hours payments *

I agree that my service must be paid in full by the end of my appointment. Installments are cash only. If booking after hours I accept my appointment will have a 50% increase in price.

Parking *

I agree and understand that the field accross the street is private property. I will only park in the driveway.

Photo Release & Waiver *

I agree that the owner may take pictures or video of my appointment to later be posted online.

Please, no photos.

Release of Liability (View terms of use under signature section below) *

I hereby release the owner (Amber Buti of Butiify), its stylists, colorists, and employees, from any responsibility and/or liability concerning the application, processing, and/or consequences of the permanent chemical procedure of my hair.

Cancellation Policy *

I understand and agree that the cancellation policy is 24-48 hours before the appointment. All of the protocols are listed in the email upon booking. I acknowledge that I will be sent a fee of either 50% or 100% of my services if I cancel within the 24-48 hour window.
